Knights News · 2020-2021 Athlete/Parent Paperwork Requirements


All sports will be allowed to start practices and conditioning the week of July 6, 2020 according to the recent release from the Indiana Department of Education.   Additional information will be coming in the following weeks as it becomes available with more details about dates, times, restrictions, etc.

Several items MUST be completed on Rank One by athletes and parents prior to participating in any workouts.

1.  IHSAA physical must be on file.  Athletes who had a completed physical on file at Castle High School, Castle North Middle School, or Castle South Middle School for the 2019-2020 season are not required to complete another physical for the 2020-2021 season.  However, there is a Health History form that needs to be completed on each student’s Rank One account allowing last year’s physical to be used.  (All physicals collected from CHS, CNMS, and CSMS for 2019-2020 have been checked off for the 2020-2021 season.  If you feel there is an error and the physical is not checked off in your account, please contact the athletic director.  There were several physicals from the middle schools that were collected that were incomplete and can’t be used until all fields on the physical are complete.)

2.  The Online Signature Page includes the Warrick County School Corporation Code of Conduct and IHSAA Concussion and Sudden Cardiac Arrest acknowledgement forms.

3.  The Emergency Medical Contact page provides us with pertinent information should we need to contact someone in case of an emergency.

These forms can be found at Rank One Sport.  Students will use their student ID numbers and DOB to log in.  Students who are new in the Warrick County School Corporation will not be in the system and need to contact Athletic Director Brandon Taylor at btaylor@warrick.k12.in.us to set up an account.

ALL OF THESE MUST BE COMPLETED PRIOR TO ANY WORKOUTS FOR THE 2020-2021 SEASON!

*The concussion impact test (IMP) will be conducted by the school’s certified athletic trainer.  This can NOT be completed at home and will remain unchecked until the test has been completed.
