Physical Opportunity #1 Tri-State Orhopaedics will be offering sports physicals on Saturday, May 3 from 8am-1pm at their office located at 225 Crosslake Drive in Evansville. Physicals are $10 and all money will be donated back to the school's athletic department. Bring the completed Preparticipation Physical History Form with you to expedite the process. Once they physical is completed, you will upload the Physical Examination (page 3) to Final Forms to be approved for next school year.
